PURPOSE: This study aimed to explore the experience of clinical nurses regarding training programs for critically ill patients with coronavirus disease 2019 (COVID-19) and their educational needs. METHODS: Qualitative data were analyzed using content analysis, and quantitative data were analyzed according to Borich's formula. Data for the study were collected in March 2021 from 16 nurses who had completed a nursing program for critically ill patients with COVID-19 and were working at three hospitals designated for COVID-19. RESULTS: Participants' experiences were classified into three major categories, namely "Participation experiences and perceptions of the training program," "Recommendations for improving the training program," and "Perceptions of working in an infectious environment," and 10 subcategories. According to Borich's formula, the most pressing educational needs in respiratory and non-respiratory nursing, respectively, were for "nursing care for patients on extracorporeal membrane oxygenation" and "application of continuous renal replacement therapy and caring for patients." CONCLUSION: To prepare for the periodic emergence of communicable infectious diseases throughout the world and cultivate nursing staff to care for critically ill patients, it is necessary to develop nursing education programs with content corresponding to nurses' needs. This study can be used as base data for cultivating nursing staff for critically ill patients with communicable infectious diseases in keeping with clinical nurses' educational needs and basic educational materials for nursing students. Clinical nurses have training needs related to the strategies for infection spread prevention. This study aimed to investigate the levels of importance of and performance in the various areas of care for communicable respiratory infections among clinical nurses and to determine the priority of educational needs. Hospitals in which a baseline survey could be conducted were considered, and nurses working in six hospitals that were designated as COVID-19 care centers in Korea were enrolled. The training needs for the care of patients with communicable respiratory infections were analyzed using Borich's needs equation, and the locus for focus model. Among participants with prior COVID-19 patient care experience, according to Borich's equation, the need score was the highest for "initial response to communicable respiratory infection", followed by "management of aerosol-generating procedures in patients with communicable respiratory infection" and "reporting of patients with communicable respiratory infection and death of patient". An item rated highly in both Borich's equation and the locus for focus model among the participants with prior experience was "initial response to communicable respiratory infection". Among participants without prior relevant experience, according to Borich's equation, the need score was highest for "management of aerosol-generating procedures in patients with a communicable respiratory infection", followed by "initial response to communicable respiratory infection" and "reporting of patients with communicable respiratory infection and death of patient". None of the items were rated high in both Borich's equation and the locus for focus model among participants without relevant prior experience.

Nurses have experienced unintended consequences and workarounds associated with health information technology implementation. However, examination of this occurrence is rare. This study aimed to examine the unintended consequences and workarounds produced by the implementation of electronic medical record systems in clinical nursing practice. A total of 143 nurses participated in a survey using statistically tested instruments. The data were analyzed using descriptive statistics and a nonparametric test. The descriptive data were analyzed by meaning. The participants experienced unintended consequences and workarounds related to electronic medical record implementation at moderate or high levels based on the responses to questions scored on 5-point Likert scales. The unintended consequences were closely associated with workarounds. The degree of experience with unintended consequences and the use of workarounds differed significantly according to the level of education, job position, and years in nursing practice. The nursing examples of unintended consequences and workarounds were organized into four categories of unintended consequences. By presenting unintended consequences and workarounds together, this study enhances the understanding of the problems encountered in EMR implementation and the action of nurses. Nurses' needs should be considered as an important resource in developing, redesigning, or purchasing and implementing health information technology in healthcare settings.

Visual display terminal syndrome is a health problem that occurs when an individual looks at a visual display terminal for a long time. This study aimed to evaluate the quality of YouTube videos on visual display terminal syndrome. YouTube was searched using the keywords "visual display terminal syndrome" and "a prolonged user of a computer or smartphone" on October 16, 2019. A total of 45 videos were evaluated using DISCERN and the Journal of the American Medical Association scoring systems. The variables extracted from the videos were the uploading agency; content; presentation format; days since upload; the number of views, likes, dislikes, and comments; and the video power index. The mean DISCERN and Journal of the American Medical Association scores were 35.64 and 3.08 points, respectively, indicating that the information on visual display terminal syndrome in YouTube videos was inaccurate and unreliable. The major reason for the low quality of the videos was that the sources of information presented in the videos were not provided. The DISCERN and Journal of the American Medical Association scores showed significant differences in the uploading agency and presentation format variables. Nurses must be familiar with evaluating the quality of videos presenting health information. Guidelines informing patients that YouTube might provide misinformation about health need to be developed.

The purpose of this study was to translate the Pregnancy Physical Activity Questionnaire, a semi-quantitative tool that asks participants about time spent on 32 activities, into Korean and verify its validity and reliability. In total, 363 pregnant women under prenatal care at an obstetrics and gynecology hospital and a postpartum care facility in Gyeonggi-do completed the Korean version of the Pregnancy Physical Activity Questionnaire. The questionnaire's content validity, construct validity, concurrent validity, and reliability were verified. After verifying the validity of the contents of the Pregnancy Physical Activity Questionnaire, all the questions were included in the Korean version. For construct validity, we divided the participants into primipara and multipara groups based on their delivery history. On comparison of the two groups' physical activity based on the responses to the Pregnancy Physical Activity Questionnaire, there was a statistically significant difference in the total activity (t = -4.56, p < 0.001) and the total activity (light activity or more) (t = -5.80, p < 0.001). The correlation between the Pregnancy Physical Activity Questionnaire and the Global Physical Activity Questionnaire was tested to establish concurrent validity, and a significant correlation was found between all items except for vigorous physical activity. The Guttmann reliability coefficient by the odd-even method was 84. The Korean version of the Pregnancy Physical Activity Questionnaire is a suitable tool to measure the physical activity of pregnant women and can be used in clinical practice.

RESUMO

This study examined the reliability and validity of the Korean version of the anesthesia surrendering instrument (ASI), which was originally developed to measure anesthesia surrendering in Swedish adults. The study population consisted of 306 patients who received general anesthesia for abdominal, breast, knee, hip, lower back, or shoulder surgery in ten hospitals across five regions of Korea from June to September 2019. The validity of the content, construct, and criterion used, and the reliability of the ASI were assessed. The results showed that the instrument had appropriate content validity; the item-level content validity index ranged between 0.80 and 1.00, and the scale-level content validity index was 0.90. The construct validity test results confirmed four sub-categories with a total of 26 items, and the internal consistency reliability tests showed Cronbach's alpha values ranging between 0.71 and 0.88. The study findings confirmed the applicability of this instrument for measuring anesthesia surrendering in Korean adults. These results provide a foundation for future studies on anesthesia surrendering in Korean adult patients.

PURPOSE: The population of metabolic syndrome is increasing in Korea. The prevalence is related to lifestyle, such as lack of physical activity and irregular diet. Most patients with metabolic syndrome know that lifestyle intervention is important to managing the condition. However, they do not always follow the intervention for various reasons, including lack of knowledge on how to change their lifestyle and lack of accessibility to that knowledge. The purpose of this study was to test the web-based health promotion program we developed. METHODS: Fifty-six adult workers from eight areas of business were recruited. They all had a confirmed metabolic syndrome diagnosis after being registered at a university hospital for annual health checkups. Twenty-nine workers were assigned to the experimental group, and the others were assigned to the control group. The web-based program was applied to the experimental group as an intervention for 8 weeks. Waist circumference, fasting blood glucose, triglyceride, high-density lipoprotein cholesterol, and blood pressure were measured before and after the program. Data were analyzed using chi-square test, Fisher's exact test, and t test. RESULTS: There were statistically significant differences between the two groups in waist circumference and high-density lipoprotein cholesterol (t = -4.43, p < .001; t = 2.22, p = .031, respectively). Of the 29 participants, 13 (44.83%) in the intervention group had less than two indices for metabolic syndrome. CONCLUSIONS: This study suggests that a web-based program is useful for patients with metabolic syndrome to improve physiologic parameters related to metabolic syndrome. The web-based program may be easily applicable to community as well as clinical setting.

PURPOSE: The prevalence of urinary incontinence and the usage of incontinence pads by elderly residents in Korean medical welfare facilities were investigated. METHODS: From a total of 1,832 facilities, 33 medical welfare facilities in the capital area were randomly selected; all nursing homes were excluded. All data were collected by questionnaires to investigate the residents' age and sex, the presence or absence of urinary incontinence, incontinence pad usage per month, and management methods for urinary incontinence. Surveys were also conducted from January 2010 to August 2010. RESULTS: A total number of residents in medical welfare facilities were 2,783, and all of them were selected for this study. Approximately, 65.3% of residents (1,816 individuals) had incontinence. The mean usage was 75 incontinence pads per month. Only 15.6% of residents received proper management for urinary incontinence, while the remaining 84.4% of residents did not have any experience in evaluating or managing their urinary symptoms. CONCLUSIONS: The prevalence of urinary incontinence in medical welfare facilities in Korea was about 65.3%. However, the management of urinary incontinence was insufficient. Urologists should make further efforts for the proper management of urinary incontinence in elders in these facilities.

AIM: To examine the effects of Meridian acupressure on the functions of affected upper extremities, activity of daily living and depression for stroke patients. BACKGROUND: The handicaps of stroke patients necessitate multidisciplinary rehabilitation, which is aimed at decreasing the consequences of the illness in daily functions. In traditional Chinese medicine, Meridian acupressure has been used for rehabilitation of affected upper extremities of hemiplegia stroke patients in clinical practice. DESIGN: Randomised clinical trial. METHOD: Sample was 56 consecutive stroke patients in K Oriental hospital. Meridian acupressure was applied every day for 10 min per time in the experimental group (28 subjects) for two weeks. Routine care only was applied in the control group (28 subjects). Measures were a hand dynamometer for grip power, a 15-point graphic rating scale for pain, the distance round the index finger for oedema, electrogoniometry for the passive ROM of the upper extremities, the activity of daily living scale for activity of daily living and the six-face rating scale for depression. Data were collected from March 2003-September 2004. RESULTS: There were significant differences in functions of affected upper extremities, activity of daily living and depression between experimental and control group. CONCLUSIONS: Meridian acupressure was an effective intervention for improving the movement of the affected upper extremities, increasing activity of daily living and decreasing depression of hemiplegia stroke patients. RELEVANCE TO CLINICAL PRACTICE: Health professionals need to consider an array of methods including Meridian acupressure for the rehabilitation of stroke patients. Meridian acupressure has the potential as an alternative nursing intervention for multidisciplinary rehabilitation of stroke patients in practice.
